Join North York General to accelerate your career Position Summary Clinical Team Manager, Integrated Transitions Reporting to Program Director, Transitional Care, Community Integration & Ambulatory Care, your goal in this position is to ensure operational and clinical excellence driving Integrated Care strategic direction; while facilitating the provision of an outstanding patient and family experience, as patients transition to their next health care journey. The Clinical Team Manager is responsible for supporting the development of a high performing Senior Friendly team, ensuring the delivery of high quality and safe care for patients, caregivers and families, and a healthy work environment for staff, physicians and volunteers. The role will involve the oversight and understanding of timely transitions in care and discharge planning (including ALC) to support our patients to age at home for as long as possible. Specifically, the Clinical Team Manager is responsible for patient care delivery, operational planning, human resource development, financial management, quality management, education, and research for programs: NYG@Home, NY CARES, and North York Congregate Access & Support Team (including Nurse Led Outreach Team). In addition to programs, also lead community outreach and build capacity, and support transitions of complex patients.The Clinical Team Manager brings a high level of operational expertise and leadership skills to a diverse interprofessional care team straddling NYGH & our Ontario Health Team (North York Toronto Health Partners). All roles are essential in providing a safe and healthy environment for patients, staff, physicians, volunteers, and others by ensuring that health and safety policies are followed. You will be responsible for promoting best practices and evidence-based care through the implementation of initiatives that are aligned with the organization’s strategic direction. The position supports the hospital’s academic mandate through teaching and learning and/or transforming care through research and innovation. This role is also key to NYGH’s Ontario Health Team mandate to create a more Compassionate North York, improving population health for our community through more coordinated, integrated care across the continuum. The incumbent will be an experienced healthcare professional with demonstrated ability to collaborate with internal and external partners, have strong community and hospital understanding and operational experience and has a passion for geriatric care. They respect privacy and confidentiality. • Decisive – at the end of the day, they have the ability to make the big decisions, to take ownership of their actions, and work in the best interests of the hospital as a whole• Safety Conscious – they know that keeping everyone safe and healthy is the highest priority, and follow all H&S policies and guidelines to provide a healthy environment for patients, staff, physicians, volunteers and families On a practical level, you will Provide leadership and accountability at clinical and operational level for a high quality and safe patient/family experience (through virtual and in person management).Promote best practices and evidence-based care through the implementation of initiatives that are aligned with the organization’s strategic direction.Manage on a day-to-day basis the physical, financial and human resource of the clinical areas in line with the mission, vision, values and strategic direction of the organization.Ensure clinical and regulatory compliance of the team by supporting inter-professional practice improvement through virtual and in person visits.Maintain compliance with required Health & Safety, environmental and other legislativerequirements.Build relationships with peers and other contacts across the organization and externally to create opportunities for synergy/integration.Demonstrate leadership, with a lens of strong patient/family and service focus.Have Knowledge of applicable legislation and regulations, related both to employment law and areas of functional responsibility. Qualifications Regulated Health Care Professional in good standing with respective health professional regulatory body in OntarioUniversity degree in a health-related field, and Master's level prepared (Health Administration or Business Administration preferred)A minimum of (5) five years of clinical experience in community care and/or long-term care A minimum of (3) three years management experience with direct people management responsibilitiesSolid understanding of ALC, transitions of care, discharge planning, community supports.Experience driving integrated care and working within Ontario Health TeamDemonstrated leadership potential, with a lens of strong patient/family focusExceptional analytical and information-seeking skills that contribute to effective decision-making and strategic planning and execution of action plans that deliver results and motivate individuals for greater performance excellenceAbility to work with a diverse group of leaders and stakeholders, specifically inter-sectoral, with demonstrated success in moving key stakeholders to positive resolution and to facilitate and coach clinical areas on change initiatives Strong project management skills with demonstrated experience in leading a team through change, with successful outcomes Extensive change management experience, and the ability to adapt or be flexible, and is comfortable with ambiguityHighly effective interpersonal and communication (spoken and written) skills, combined with excellent organizational skills, with demonstrated ability to achieve outcomes Good understanding of professional practice standards Strong people skills, with ability to give feedback Strategic conceptual thinker, with ability to integrate systems thinking into operations Knowledge of applicable legislation and regulations, related both to employment law and areas of functional responsibility .
