Join North York General to accelerate your career Position Summary The Clinical Team Manager, Integrated Transitions reports to the Program Director, Transitional Care, Community Integration & Ambulatory Care. Your goal is to ensure operational and clinical excellence driving integrated care strategic direction while facilitating an outstanding patient and family experience during transitions. The role involves overseeing patient care delivery, operational planning, human‑resource development, financial and quality management, education and research for programs such as NYG@Home, NY CARES, and North York Congregate Access & Support Team. You will lead community outreach, build capacity, and support transitions of complex patients, collaborating across NYGH and the Ontario Health Team. On a Practical Level Provide leadership and accountability at the clinical and operational level for a high quality and safe patient/family experience (through virtual and in‑person management). Promote best practices and evidence‑based care through the implementation of initiatives aligned with the organization’s strategic direction. Manage day‑to‑day physical, financial and human resources of the clinical areas in line with the mission, vision, values and strategic direction. Ensure clinical and regulatory compliance of the team by supporting inter‑professional practice improvement. Maintain compliance with required Health & Safety, environmental and other legislative requirements. Build relationships with peers and other contacts across the organization and externally to create opportunities for synergy and integration. Demonstrate leadership with a strong patient/family and service focus. Have knowledge of applicable legislation and regulations, both employment law and functional responsibilities. Qualifications Regulated Health Care Professional in good standing with the respective health professional regulatory body in Ontario. University degree in a health‑related field; Master’s level preferred (Health Administration or Business Administration). Minimum of five (5) years clinical experience in community care and/or long‑term care. Minimum of three (3) years management experience with direct people‑management responsibilities. Solid understanding of ALC, transitions of care, discharge planning and community supports. Experience driving integrated care and working within an Ontario Health Team. Demonstrated leadership potential with a strong patient/family focus. Exceptional analytical and information‑searching skills for effective decision‑making and strategic planning. Ability to work with diverse leaders and stakeholders, including inter‑sectoral collaboration. Strong project‑management skills with demonstrated experience leading a team through change. Extensive change‑management experience and comfort with ambiguity. Highly effective interpersonal and communication skills, both spoken and written. Good understanding of professional practice standards. Strong people skills and ability to give feedback. Strategic conceptual thinker with ability to integrate systems thinking into operations. Knowledge of applicable legislation and regulations (employment law and functional responsibilities).
