This role is ideal for someone who is passionate about helping clients remain safe, mobile, and independent in their own homes. You will work under the supervision and direction of a Registered Occupational Therapist and/or Physiotherapist and support clients across the Central region including York, Simcoe, Marham, Newmarket, Geogina, and Alliston.While we thank all interested applicants, we are only able to contact those who we are inviting to an interview. Key Responsibilities: Implement prescribed treatment plans under the direction and supervision of a Registered Occupational Therapist and/or Physiotherapist Support clients with exercises, mobility training, strengthening programs, gait training, balance retraining, and functional mobility tasks, as well as other prescribed activities Reinforce home exercise programs and ensure clients perform exercises safely and correctly Monitor and document client progress, observations, and responses to treatment (with OT/PT review and oversight) Assist with transfer training, safe lifting techniques, and mobility aid usage Provide caregiver education and support for OT/PT-directed care strategies Assist with basic equipment setup, environmental safety recommendations, and home visit follow-up tasks Maintain appropriate communication with supervising OT/PT, office staff, and interdisciplinary team members Travel to client homes across the GTA and maintain flexibility as caseload changes RequirementsMUST have a valid driver's license and reliable access to a car that can be used for daily workAbility to travel between assignments within designated areas of work, an asset if you can travel to rural/remote areas.Must be able to communicate effectively both verbally and in writing in EnglishPSW certification from an approved and recognized educational institution or International nursing/ doctorate CertificationNegative TB Test completed within the last yearValid CPR/First Aid CertificateValid WHMIS2015 Certificate of CompletionCOVID Vaccination Receipts subject to an exemption pursuant to Ontario’s Human Rights CodeImmunization for measles, rubella, varicella, tetanus, and Diphtheria REQUIRED, immunization of Hepatitis B is strongly urged.Valid Criminal Record Check - must be completed within the last year.Must be able to communicate effectively both verbally and in writing in EnglishAbility to identify and report client care concerns to Care Coordinators when needed.Requires good judgement, the ability to work independently, and work cooperatively with an interdisciplinary teamCurrent/Valid Health and Supportive Care Providers Oversight Authority (HSCPOA) Registration an asset*The Care Company can make accommodations/modifications for individuals with disabilities during the hiring and onboarding process.
