Join North York General to accelerate your career Position Summary Clinical Team Manager, Reactivation Care Centre Your goal in this position is to ensure operational and clinical excellence, while facilitating the provision of an outstanding patient and family experience transitioning patients in their health care journey. The Clinical Team Manager is responsible for supporting the development of a high performing Senior Friendly team, ensuring the delivery of high quality and safe care for patients and families, and a healthy work environment for staff, physicians, and volunteers. The role will involve the oversight of Alternative Level of Care (ALC) patients which will require an understanding of timely transitions in care and discharge planning. The Clinical Team Manager brings a high level of operational expertise and leadership skills to a diverse care team. All roles are essential in providing a safe and healthy environment for patients, staff, physicians, volunteers, and others by ensuring that health and safety policies are followed. You will be responsible for promoting best practices and evidence-based care through the implementation of initiatives that are aligned with the organization’s strategic direction. Information flows freely, allowing their team to operate at a high level, and their patients to feel cared for and secure• Decisive – at the end of the day, they have the ability to make the big decisions, to take ownership of their actions, and work in the best interests of the hospital as a whole• Safety Conscious – they know that keeping everyone safe and healthy is the highest priority, and follow all H&S policies and guidelines to provide a healthy environment for patients, staff, physicians, volunteers, and families On a practical level, you will Provide leadership and accountability at clinical and operational level for a high quality and safe patient/family experience.Manage on a day-to-day basis the physical, financial, and human resource of the clinical areas in line with the mission, vision, values, and strategic direction of the organization.Lead and support the interprofessional team in promoting and prioritizing discharge planning and care transitions to enable efficient patient flow.Provide leadership and promote best practices and evidence-based seniors care through the implementation of initiatives in alignment with Regional Geriatrics Program RGP, local Ontario Health Team (OHT), and regional strategic priorities Lead and contribute to continuous quality improvement initiatives within and outside the organization Establish relationships with the RGP and other community partners, and organizations, to enhance the circle of care for complex elderly clients.Ensure clinical and regulatory compliance of the team by supporting inter-professional practice improvement through virtual and in person visits.Maintain compliance with required Health & Safety, environmental and other legislative requirements.Build relationships with peers and other contacts across the organization and externally to create opportunities for synergy/integration.Demonstrate leadership, with a lens of strong patient/family and service focus.Have Knowledge of applicable legislation and regulations, related both to employment law and areas of functional responsibility. Qualifications Registered Nurse with Baccalaureate DegreeRegistration in good standing with respective Professional College Masters degree in relevant field required (or, currently within Masters Program)3 to 5 years clinical management or leadership experience in community care and/or hospital Demonstrated leadership potential, with a lens of strong patient/family focusManagement and supervisory experienceStrong project management skills with demonstrated experience in leading a team through change, with successful outcomesHighly effective interpersonal and communication (spoken and written) skills, combined with excellent organizational skills, with demonstrated ability to achieve outcomesGood understanding of professional practice standardsGood understanding of ALC, transitions of care, discharge planning, and community supportsGood understanding and experience of restorative and rehabilitative care for the elderly, and palliative care services Strong people skills, with ability to give feedbackStrategic conceptual thinker, with ability to integrate systems thinking into operationsAbility to work with a diverse group of leaders and stakeholders, specifically inter-sectoral, with demonstrated success in moving key stakeholders to positive resolution and to facilitate and coach clinical areas on change initiatives Knowledge of applicable legislation and regulations, related both to employment law and areas of functional responsibility.
