At Community Advantage Rehabilitation, we work together with our clients to advance their quality of life using best practices, innovation and quality standards.PT Responsibilities:Provide ministry funded, community based Physiotherapy services within the pediatric/young adults client’s place of residence and/or through virtual servicesProvide physiotherapy assessment (through interview, standardized and informal assessment techniques) in the areas of gross motor skills, co-ordination, neurological, developmental, mobility aids, etc.Develop and implement goal-oriented treatment plans in collaboration with clients/caregiversEffectively communicate information by providing education to client/caregivers and linking to community agencies/resources when appropriateComplete and maintain current and accurate documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and College of Physiotherapists of Ontario standardsOpportunity to service private clientsRequirements:Registered Physiotherapist with the College of Physiotherapists of Ontario (required)Experience working within a community setting is an asset but new graduates are welcomeAbility to work independently and as part of an inter-professional teamAbility to prioritize, manage self-scheduling and meet reporting deadlinesExcellent oral, written and report writing skillsProficiency with computersStrong interpersonal and organizational skillsReliable transportation is required for this positionVisit us on -Apply todayInclusionWe are committed to creating an inclusive environment where people from all backgrounds can thrive.
