Indigenous Transition Facilitator

The Indigenous Transition Facilitator supports self-identified First Nations members from one of Sioux Lookout First Nation Health Authority’s member communities. The ITF supports transition planning to ensure access to equitable care while navigating the provincial and federal healthcare systems. The ITF will help First Nation members access healthcare outside of their community as well as within hospitals. The ITF supports transition into care and back home.
The Indigenous Transition Facilitator will report to the Director Health Benefits & Client Support.
Qualifications:
- Persons of First Nations ancestry will be given preference (OHRC, Part II, Special Employment).
- Ability to communicate in one of the First Nations dialects in the Sioux Lookout Zone is an asset.
- Registered Nurse or Practical Nurse in good standing with the College of Nurses of Ontario OR Social Worker in good standing with the OCSWSSW
- An additional qualification in Community and Health Services Navigator College Certificate is an asset.
- Knowledge of health system navigation, community resources, legislation, research and funding agreements specific to Indigenous programs and services
- Proficiency in the use of computers and various database software applications including electronic health records, MS Word, Excel, and scheduler programs
- Problem-solving, good judgement and decision-making skill and ability in times of crisis
- Experience in an Indigenous health and/or social services organization, hospital, or community-based case management, health promotion, public health or social work is essential
- Ability to prioritize professional duties, manage multiple patients, and efficiently organize a caseload
- Strong interpersonal and collaboration skills
- The ability to perform the requirements of the position on a regular basis.
Roles and Responsibilities:
- Development and Implementation of discharge planning models such as Patient Oriented Discharge Summary (PODS) or the Winnipeg Regional Health Authority Discharge Planning to First Nation Communities model
- Ensure early engagement of the client and family (as identified by the client), as well as with interprofessional team to develop effective transition plans. Develop a patient-centered transition plan that considers multifaceted needs of the patient and family and actively engages them as partners.
- Together with the interprofessional team and the patient/family, work to solve problems and overcome barriers to the transition process.
- Facilitate and take the lead on all aspects of the transition process in collaboration with the interprofessional team and engage all key stakeholders required
- Partner with community resources from public/private and non-profit/charitable agencies to build sustainable transitions
- Ensure medication reconciliation and medication teaching have been completed with client and family prior to discharge, and the Community Health Representative (or other human resources) in the community is aware of any changes to medications upon the patient return to community
- Ensure follow up appointments are booked and Non-Insured Health Benefits (NIHB) medical transport is aware of upcoming appointments
- Knowledge and understanding of Non-Insured Health Benefit programs and services. Ability to advocate and support the client to meet their needs.
- Liaise with NIHB to ensure ordering of medical equipment and supplies on behalf of the client
- Ensure medical translation services are available and that the client and family have received the education and resources they require for the best possible client experience. Ability to provide medical translation services for Indigenous languages an asset
- Facilitate culturally appropriate patient care
- Willingness and capacity to develop relationships with the 33 First Nation communities serviced by SLFNHA
- Attend and participate in the Indigenous Transitions Facilitator Community of Practice on a quarterly basis
- Record appropriate statistical data and develop reports as needed
- Manage and assist in managing confidential client information
- Other duties as required
