About the Role

We are seeking a skilled Senior Clinical Social Worker to join our team at Fraser Health. This is an exciting opportunity for a highly motivated and experienced professional to contribute to providing high-quality care to patients and their families.

Responsibilities

  • Conduct comprehensive psychosocial assessments of clients, identifying areas of strength and challenge in addressing emotional and social components of illness, injury, or disease.
  • Provide evidence-based therapeutic clinical counseling services to promote improved emotional well-being and resilience.
  • Collaborate with interdisciplinary teams to develop and implement effective care plans, ensuring seamless transitions and optimal outcomes.
  • Communicate effectively with clients, healthcare professionals, and community agencies to ensure comprehensive support and resource utilization.
  • Maintain accurate and timely documentation of client records, progress notes, and reports, adhering to established professional and organizational standards.

Requirements

  • Bachelor's Degree in Social Work from an accredited institution.
  • Current full registration with the BC College of Social Workers.
  • At least one year of recent experience in a related field, with a strong focus on clinical practice and interpersonal skills.
  • Knowledge of relevant legislation, acts, and current social work practices in health care, including AGL, Mental Health, Child Protection, FOI, Organ Donation, and chemical dependency.
  • Excellent communication, collaboration, and time management skills, with the ability to work independently and as part of a multidisciplinary team.
