We are seeking a highly skilled Clinical Social Worker to join our team at pt Health. This is an excellent opportunity for a dedicated professional to work with our clients and make a positive impact on their lives.


Key Responsibilities



  • Provide goal-focused psychotherapy to address mental health and psychosocial problems.
  • Conduct social work assessments and interventions to improve social and community functioning.
  • Utilize a cognitive-behavioral approach to intervention based on a client-centered philosophy.
  • Provide group psychoeducational services.
  • Participate in interdisciplinary team conferences to discuss treatment plans and goals.
  • Deliver quality reporting and effective communication with stakeholders.

Requirements



  • Masters Degree in Social Work (or equivalent)
  • Registration with a provincial College of Social Workers as an autonomous practitioner
  • 2 years of experience in case conceptualization and psychotherapy/social work intervention
  • Experience providing social work services to military, first responders, and veterans an asset
  • Experience working as part of a multi/interdisciplinary team
  • Excellent interpersonal skills, autonomy, and integrity
