Key Responsibilities- Manage and coordinate care, evaluating health outcomes and educating clients and families on health goals.
- Provide all levels of nursing care, from stable clients with predictable outcomes to those with acute, complex, and rapidly changing health needs.
- Assign and delegate care as necessary, ensuring cost-effective and organized care.
- Develop and implement care plans with clients and families, incorporating discharge planning as required.
- Implement nursing strategies and interventions according to established care plans.
- Document nursing processes clearly and accurately, adhering to regulatory and VON standards.
- Evaluate client responses to care plans and revise plans as necessary.
- Promote client engagement in care and support independence.
- Negotiate with clients and families to ensure effective care delivery while respecting client choices.
- Stay current with research findings and incorporate them into clinical practice.
- Collaborate with other healthcare professionals and the community to promote nursing best practices.
- Licensure as a Registered Nurse in the province of practice.
- Canadian Nurses Association certification in Community Health Nursing is an asset.
- Clinical practice experience and experience in community health are assets.
- Strong written and verbal communication skills, with the ability to engage with clients, staff, and the public.
- Strong interpersonal relationship building skills and the ability to work collaboratively as a member of an interdisciplinary team.
- Ability to problem-solve and adapt nursing interventions as necessary.
This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying, bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in this role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
