Complex Care Clinician

Vancouver Coastal Health is seeking a skilled Registered Nurse to join our Home Health team as a Complex Care Clinician. In this role, you will work with high-complex medically frail clients, providing specialized care and support to help them maintain their independence and quality of life.
Key Responsibilities- Apply specialized theory, clinical expertise, and knowledge to address the complex medical, cognitive, and behavioral needs of eligible clients.
- Develop and implement individualized care plans that prioritize client-centered care and promote optimal health outcomes.
- Collaborate with interdisciplinary teams, medical providers, and community agencies to ensure seamless care coordination and support.
- Provide wrap-around care to designated client populations, focusing on prevention, chronic disease management, and palliative care.
- Conduct ongoing assessments, identify potential issues, and develop strategies to address them in collaboration with the client and their care team.
Home Health programs provide a range of health care and support services to individuals living in the community who require ongoing care and support. Our team works with clients who have acute, chronic, palliative, or rehabilitative health care needs, providing personalized care and support to help them maintain their independence and quality of life.
Qualifications- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two years' recent case management or home care nursing experience, including experience working with complex medically frail populations.
- Valid BC Driver's License and reliable transportation.
