About the Role

At Forest View Hospital, we are seeking a dedicated and compassionate Patient Care Coordinator to join our team. This is a per diem position, allowing for flexible scheduling and work-life balance.

Job Summary

The Patient Care Coordinator provides high-quality patient care in a psychiatric setting, working closely with patients, families, and healthcare professionals to ensure seamless transitions and optimal outcomes.

Key Responsibilities:

  • Develops and implements individualized care plans to meet patients' unique needs and goals.
  • Fosters strong relationships with patients, families, and interdisciplinary teams to promote effective communication and collaboration.
  • Collaborates with healthcare professionals to coordinate discharge planning, ensuring timely and safe transitions to community settings.
  • Provides education and support to patients and families on mental health conditions, treatment options, and coping strategies.
  • Maintains accurate and detailed records of patient interactions, progress, and outcomes.

Qualifications

  • Certified Nursing Assistant (CNA) or Licensed Practical Nurse (LPN) with current licensure in Michigan.
  • Minimum one year of experience in psychiatric nursing or a related field.
  • Strong communication and interpersonal skills, with ability to work effectively with diverse populations.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Familiarity with electronic medical records and other software applications used in healthcare settings.
