Patient Care Coordinator

We are seeking a highly organized and detail-oriented Patient Care Coordinator to join our team at Home Instead. As a Patient Care Coordinator, you will play a vital role in coordinating the movement of patients through the healthcare system, ensuring that quality nursing care is provided, and setting up and coordinating nursing services in conjunction with other health services.
Key Responsibilities:- Coordinate patient care services, ensuring seamless transitions and effective communication between healthcare providers.
- Develop and maintain relationships with healthcare providers, community resources, and patients to ensure comprehensive care.
- Assess patient needs and develop personalized care plans, ensuring alignment with healthcare goals and objectives.
- Collaborate with healthcare teams to ensure timely and effective care delivery, including medication management and treatment plans.
- Monitor patient progress, identifying areas for improvement and implementing changes as needed.
- Bachelor's degree in a related field or equivalent experience.
- 1-2 years of experience in patient care coordination or a related field.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects.
- Ability to maintain confidentiality and handle sensitive information with discretion.
