The Role Involves
- Developing and maintaining an effective Care Plan with respect to Community Health Services
- Working with patients to provide exceptional nursing services
- Providing services in various settings, including in-home, clinic, retirement home, or school
- Communicating and liaising with patients, family members, and the interdisciplinary care team
- Performing other clinical duties as required
What You Bring
- Current College of Nurses of Ontario (CNO) registration in good standing
- A strong desire and commitment to making a difference in the lives of our patients
- Exceptional interpersonal skills
- A valid driver's license and vehicle, as regular travel is required
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record, including a 2-step TB test
- A clear background check, to protect our patients
CarePartners In Your Community
We serve the community through clinics, transitional care units, and provide relief in retirement homes and shared care settings. Our Community Nursing Services outreach program has been organizing staff-led medical care and clinics in countries with poor access to healthcare since 2009.
