At ComForCare Home Health Care - Kitchener/Waterloo, we are seeking a skilled Nurse Care Specialist to join our team. As a Comprehensive Care Coordinator, you will be responsible for conducting thorough client assessments, creating personalized care plans, and implementing interventions to achieve client goals.

Key Responsibilities:

  • Perform comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
  • Develop an individualized plan of care for each client that incorporates analysis of data and current scientific findings.
  • Provide interventions with a focus on achieving realistic client outcomes.
  • Report changes to update the plan of care to reflect progress towards goals and outcomes.
  • Administer medications as needed.
  • Maintain client records showing systematic assessments, planning interventions, and evaluations.
  • Initiate referrals as needed to other health team members.

Requirements:

  • Graduate of, at a minimum, a Practical Nursing Program (2-year college certificate)
  • Registered with the College of Nurses
  • Minimum of one (1) year experience in an acute care setting or equivalent experience
  • CPR certified
  • Demonstrated written, verbal, and interpersonal communication skills
  • Licensed driver with automobile insured in accordance with provincial and/or Agency requirements

Salary Range:

  • $24/hr - $25/hr
