Job Summary

CarePartners is seeking a skilled Community Nurse to join our team. As a Community Nurse, you will provide exceptional nursing services to patients in their homes, clinics, and other community settings.

About the Role

  • Develop and maintain effective care plans for patients receiving community health services.
  • Provide nursing services to patients in various settings, including in-home, clinic, and retirement home settings.
  • Communicate and liaise with patients, family members, and other healthcare professionals to ensure seamless care.
  • Perform other clinical duties as required.

Requirements

  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma.
  • Current College of Nurses of Ontario (CNO) registration in good standing.
  • Strong desire and commitment to making a difference in the lives of our patients.
  • Exceptional interpersonal skills.
  • A valid driver's license and vehicle.
  • Excellent English verbal and written communication skills.
  • CPR Certification.
  • Current immunization record including 2 step TB test.
