Job Description

The Nexus Primary Care Clinic is a partnership between Island Health and the Nexus Patient and Community Care Society, operating within Nanaimo's Primary Care Network. This collaboration aims to develop a network of outstanding healthcare professionals and build healthier communities.

This role requires an Advanced Primary Care Clinician to work in a team-based model, collaborating with Registered Nurses, Allied Health Clinicians (Social Work and Mental Health and Substance Use), and a Clinical Pharmacist.

Main Responsibilities:

  • Comprehensive patient assessment, including diagnosing diseases, disorders, and conditions.
  • Initiating treatment, including health care management, therapeutic interventions, and prescribing medications.
  • Providing medical coordination and participating in multidisciplinary team planning for ongoing patient health needs.
  • Participating in the development and operation of a team-based practice-wide program, encompassing health promotion/disease prevention, illness prevention, chronic disease management, and specialty care for specific populations.
  • Clinical teaching/preceptorship.
  • Clinical administrative services, including health care/service planning activities and participation in the evaluation of service efficiency, quality, and delivery.
