About the RoleAs an integral member of our interdisciplinary team, you will be responsible for delivering person-centered care that empowers patients to achieve their goals and maintain independence in their homes. Your expertise as an occupational therapist will enable you to develop creative, tailored treatment plans that address the diverse needs of your clients.
Key Responsibilities- Assessment and Implementation: Provide comprehensive OT interventions, including ADL/IADL strategies, equipment recommendations, and home modifications to support patient independence.
- Team Collaboration: Work closely with primary care providers and other healthcare professionals to deliver seamless, interdisciplinary care.
- Holistic Care: Support a restorative approach to health, offering interventions, health coaching, and self-management support to promote overall well-being.
- Digital Health: Utilize digital platforms for care delivery, collaboration, documentation, and intervention to enhance efficiency and effectiveness.
