We are seeking a highly skilled and compassionate Registered Nurse (RN) or Registered Psychiatric Nurse (RPN) to join our team as a Geriatric Care Manager.

About the Role

The successful candidate will report to the Director of Care and be responsible for coordinating and supervising the activities and duties of those who deliver direct resident care. As a Geriatric Care Manager, you will perform duties and monitor departmental activities in accordance with established standards, regulations, policies, and procedures to maximize person-centered quality care while maintaining resident respect and dignity.

Responsibilities:
  • Assess residents' needs; implement and evaluate individualized resident-centered care plans.
  • Support Licensed Practical Nurses (LPNs) in reviewing and updating care plans as required, offering clinical decision-making support, including clinical assessments to support the care team on a daily basis as needed.
  • Administer medications and treatments as per established standards, regulations, policies, and procedures.
  • Manage community wounds, assist LPNs with assessment, treatment, and documentation as needed, facilitate referral to Wound Care Specialist when necessary. Submit wound reports to the Director of Care (DOC) or designate weekly.
  • Provide direct care to residents as necessary and liaise with other disciplines and departments to ensure quality-focused resident care.
  • Liaise with physicians, community agency representatives, families, visitors, and volunteers.
  • Support the move-in process, work collaboratively with other team members, and follow community policies and protocols.
  • Attend Care Conferences in the absence of the DOC or designate.
  • Ensure residents' rights are observed and adhere to a holistic philosophy of wellness.
  • Act as a First Aid attendant when on duty and ensure all injured staff complete required documentation.
  • Maintain a safe environment for residents, family, and staff, through accident prevention, knowledge of emergency procedures, the safe use of supplies and equipment, and reporting when repairs are necessary, and utilizing infection control procedures.
  • Coordinate and supervise care staff; direct their work and assign duties as necessary to ensure resident-focused care is provided and resident rights are observed.
  • Provide clinical direction to community care staff in the absence of the DOC or designate.
  • Fulfill clinical on-call duties as required.
  • Perform community audits and correlate data to submit to the DOC or designate.
  • Participate in the orientation of new employees.
  • Provide educational material and direction on procedures to clinical staff and demonstrate techniques.
  • Provide informed input into staff evaluations while supporting LPNs in coaching/mentoring for Health Care Assistant (HCA) staff.
  • Actively participate in the orientation of new staff, community programs, meetings, committees, and educational in-services.
  • Maintain paper and electronic documents as per established standards, regulations, policies, and procedures, ensuring confidentiality is maintained as per policies.
Qualifications and Skills:
  • Graduate of an approved program for Nursing and holds a current license with the British Columbia College of Nurses and Midwives (BCCNM) as an RN or RPN.
  • Bachelor of Science in Nursing (BSN) an asset.
  • Valid Occupational First Aid Level 1 certification and CPR.
  • Successful annual Resident Assessment Instrument (RAI) competency testing.
  • Previous experience in a long-term care facility an asset.
  • Demonstrate collaborative and team-working skills.
  • All successful applicants must pass the Vulnerable Sector Criminal Record Check applicable to Provincial guidelines.
  • Ability to read, write, and speak English proficiently and understand verbal and written instructions.
  • Computer skills, including the ability to navigate basic computer programs such as Word, Excel, Outlook.
  • Must be in good health, physically able to perform repetitive tasks, lift, squat, bend, stand, and walk as required to perform the duties of the position.
Compensation and Benefits:

We offer a competitive salary range of $80,000 - $110,000 per annum, depending on experience, plus comprehensive benefits, including medical, dental, and vision coverage, and a generous retirement plan.
