Your Opportunity:

The Alberta Health Services is seeking a skilled Social Worker II to join our team. As a Social Worker II, you will be responsible for providing a range of social work services to patients and their families, including psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Key Responsibilities:
  • Provide psychosocial assessment and resource-based interventions to patients and their families.
  • Develop and implement individualized treatment plans to address the unique needs of patients.
  • Collaborate with interprofessional team members and community agencies to ensure comprehensive care.
  • Conduct risk screening and provide counselling to patients and their families.
  • Coordinate case management and transition planning to ensure smooth transitions between care settings.
  • Develop and implement public awareness activities to promote healthy behaviors and prevent illness.
Requirements:
  • Completion of a bachelor's degree in Social Work.
  • Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Preferred Qualifications:
  • Experience working in Addiction & Mental Health.
  • Experience with Connect Care.
