About the Role:

CarePartners is seeking a skilled Physiotherapist to join our dynamic Rehabilitation Team. As a Physiotherapist - Community Care Specialist, you will play a critical role in providing high-quality patient care in the community and in patients' homes.

Key Responsibilities:
  • Conduct professional assessments of patients in the community and in their homes.
  • Develop and maintain effective Care Plans with respect to Community Health Services.
  • Work with patients to establish time-specific, measurable goals and outcomes in collaboration with the care team.
  • Provide active guidance for workplace Health and Safety.
  • Perform other duties as required.

Requirements:
  • Master's Degree (or equivalent) degree in Physiotherapy from a recognized university.
  • Registration with the College of Physiotherapists of Ontario.
  • Proven computer skills, including email, word processing, and work scheduling systems.
  • Strong assessment skills and the ability to participate in effective treatment programs.
  • Excellent English verbal and written communication skills.
  • A reliable vehicle for some regular travel.
