Job Summary

We are seeking a highly skilled Registered Nurse to join our team at Burnaby Hospital, providing exceptional care to our elderly patients in the acute care unit. As a Registered Nurse, you will be responsible for assessing, planning, and delivering high-quality patient care, collaborating with the interprofessional care team to ensure comprehensive care planning and delivery.

Key Responsibilities
  • Assess patients for their physical and emotional needs, developing and implementing individualized care plans to meet their unique requirements.
  • Collaborate with patients and their families/caregivers to develop and implement care plans, promoting person-centered care and patient autonomy.
  • Participate in the care and discharge planning process, ensuring effective communication with the interprofessional care team and Patient Care Coordinator.
  • Coordinate nursing care for patients, prioritizing tasks and delegating responsibilities to support staff as needed.
  • Collaborate with the interprofessional care team to identify and resolve issues, adjusting care plans as required and addressing changes in patient condition.
  • Teach patients and families/caregivers about illness prevention, health restoration, and health maintenance, promoting self-care and discharge planning.
  • Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
Requirements
  • Graduation from an approved school of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • One (1) year recent related acute care experience, or an equivalent combination of education, training, and experience.
  • Ability to complete initial and ongoing patient assessment (clinical and diagnostic reasoning) and provide nursing care through appropriate/prescribed technical, therapeutic, safety-type interventions.
  • Ability to communicate effectively with patients, families/caregivers, the public, medical staff, and members of the interprofessional care team using verbal, written, and electronic communication means.
  • Ability to promote person-centered care that demonstrates empathy for patients and families/caregivers, sensitivity to diverse cultures and preferences, and patient advocacy.
  • Ability to integrate and evaluate data from multiple sources to problem-solve effectively.
  • Ability to teach and instruct patients and families/caregivers about topics unique to the geriatric population essential to their health care and well-being.
  • Ability to organize and prioritize work.
  • Ability to collaborate with and promote cooperation among members of the interprofessional care team.
  • Ability to use factual information, prior learning, and basic principles and procedures to support nursing decisions and actions with relevant research-based evidence/evidence-informed practice.
  • Ability to operate related equipment, including relevant computer applications.
  • Physical ability to perform the duties of the position.
