At ParaMed Home Health Care, we're dedicated to providing the most compassionate care possible. As a Clinical Supervisor - Community Nursing Specialist, you'll play a crucial role in ensuring our team members deliver safe, quality care to clients within their community.

In this valuable position, you'll be accountable for supervising and supporting Personal Support Workers (PSWs) and Health Service Workers (HSWs), ensuring they adhere to all legislated standards of practice and company policies.

Key Responsibilities:

  • Supervise and support PSWs and HSWs to ensure compliance with legislated standards of practice and company policies
  • Respond to and advise/resolve concerns from community staff, patients/families, and other team members
  • Provide ongoing leadership through regular huddles, face-to-face meetings, and one-on-ones
  • Initiate patient safety incident investigations and document action plans
  • Work collaboratively with nursing team members to support clinical documentation and submission requirements

You'll engage patients in problem-solving using a collaborative approach, offering health teaching, building rapport, and trust to identify patient healthcare needs, strengths, capacities, and goals. You'll also engage staff in problem-solving, offering education, coaching, and mentoring in resolution of problems.

This is an exciting opportunity to develop long-term therapeutic relationships with patients, practice complex clinical skills, and participate in comprehensive training and development programs.
