The Role Involves

This position requires developing and maintaining effective care plans in conjunction with the Ontario Health atHome, working collaboratively with patients to provide exceptional nursing services, and communicating effectively with patients, family members, and other members of the interdisciplinary care team. Other duties may include performing clinical tasks as required, such as providing services in various settings and adapting to changing patient needs.
