About the Role

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Good Shepherd Centres. As a key member of our Primary Care Access and Attachment Team, you will play a vital role in providing comprehensive primary healthcare services to vulnerable populations in our community.

Key Responsibilities
  • Provide primary care to individuals served by our team, including health promotion, disease prevention, treatment of injury and illness, consultation, and support.
  • Work collaboratively with our interdisciplinary team to ensure effective communication and the timely sharing of information necessary to promote the health and wellness of our clients.
  • Coordinate and schedule service delivery between various program sites, clinic sites, and the availability of on-site physician services.
  • Provide comprehensive assessments, health examinations, and interpret diagnostic, screening, and lab tests and reports.
  • Formulate diagnoses and communicate and interpret these to clients and family members.
  • Provide care to well individuals of all ages and their families, placing emphasis on health enhancement and risk reduction.
  • Treat illnesses, within the scope of practice, through the provision of pharmacological and complimentary therapies.
  • Provide education, information, and support to individuals that supports health and wellness.
  • Utilize a holistic approach and a client-centered approach to care, paying special attention to psychological, social, cultural, and environmental factors.
  • Triage, communicate, and collaborate with hospitals and other community health services.
Requirements
  • Bachelor Degree in Nursing and Registration in the Extended Class with the College of Nurses of Ontario.
  • Two years' experience as a Nurse Practitioner, RN, (EC) preferably in a community setting.
  • Experience working with mental health, addiction, and concurrent disorders.
  • Experience and sensitivity in working with culturally diverse populations, including individuals newly arrived in Canada.
  • CPR/First Aid certification and WHMIS training and/or willingness to certify.
  • Committed to intersectional anti-racist/anti-oppressive practice, harm reduction, and trauma-informed care.
  • Ability to work both independently and as a member of a team.
  • Outgoing and adaptable to working in a fast-paced and changing environment.
