Job Title: Home Health Care Nurse

We are seeking a skilled and compassionate Registered Nurse to join our team of dedicated healthcare professionals. As a Home Health Care Nurse with RT Medical, you will have the opportunity to make a positive impact on the lives of our patients and their families.

About the Role:
  • Provide high-quality nursing care to patients in their homes, clinics, or other community settings.
  • Conduct thorough assessments, develop individualized care plans, and coordinate with other healthcare professionals to ensure seamless patient care.
  • Collaborate with patients, families, and caregivers to promote health education, disease prevention, and management.
  • Evaluate patient progress, adjust care plans as needed, and provide ongoing support to ensure optimal outcomes.
Requirements:
  • Bachelor of Science in Nursing degree (BScN) or Bachelor of Nursing degree (BN).
  • Current registration with the College of Nurses of Ontario.
  • Experience in wound care, IV medications, and re-sites, catheter care, compression/VAC, and ostomy care an asset.
  • Ability to work independently and collaboratively within a dynamic team environment.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Valid Standard First Aid and Level C CPR certification.
  • Updated immunization records and clearance for vulnerable sector check required.
