Key Responsibilities- Manage a caseload of patients with chronic kidney disease, providing comprehensive assessments, diagnoses, and treatment plans.
- Collaborate with our healthcare team to develop and implement individualized care plans, ensuring seamless transitions and continuity of care.
- Provide education and support to patients and their families, empowering them to manage their condition and make informed decisions about their care.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices in nephrology care.
- Contribute to a safe and inclusive environment for all, adhering to patient and staff safety policies and procedures.
- Registered with the College of Nurses of Ontario as an Extended Class RN and a member in good standing.
- National certification in Nephrology Nursing from the Canadian Nurses Association required or in-progress with completion date within 1 year.
- Current Basic Cardiac Life Support (BCLS) certification.
- Proven clinical experience with patient-focused care, including strong patient assessment, care planning, patient education, and documentation skills.
- Minimum of 5 years of recent clinical experience in Nephrology preferred.
- Membership in the Nurse Practitioner Association of Ontario preferred.
- Advanced clinical knowledge of current best practices in caring for nephrology/dialysis populations.
- Experience in Case Management preferred.
