About the Role

We are seeking a highly skilled and compassionate Senior Brain Injury Rehabilitation Specialist to join our team at March of Dimes Canada. As a key member of our Outreach programs, you will play a vital role in enhancing the lives of individuals with brain injuries.

Key Responsibilities
  1. Implement programs designed to promote the efficient and safe delivery of Community Support Services to clients with brain injuries, working collaboratively with the Rehab Service Coordinator (RSC).
  2. Contribute to daily report writing, incident reports, Individual Rehabilitation Plan, and quarterly reporting, ensuring accurate and timely documentation.
  3. Manage a caseload with flexibility, responding to other duties as assigned, and prioritizing tasks to meet the needs of clients and the team.
  4. Collaborate with multi-disciplinary teams to ensure individual needs are met through functional goal setting, promoting a holistic approach to rehabilitation.
  5. Act as a resource to staff and families in the provision of long-term support and service coordination for complex or catastrophic cases, empowering individuals with brain injuries to remain in their communities.
  6. Provide cognitive and compensatory strategies that support functional goal setting, helping clients to achieve their full potential.
  7. Travel within designated regions on a daily basis, ensuring effective communication and collaboration with colleagues and clients.
Requirements
  • 2-5 years of experience in the Social Services/Health Sector, coordinating services for individuals with brain injuries (an asset).
  • Concrete knowledge of the effects of an Acquired Brain Injury (ABI) is an asset, with training provided as needed.
  • Community-based experience working with individuals with brain injuries, physical disabilities, or special needs.
  • Highly organized and excellent time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Problem-solving and conflict resolution skills, with a focus on de-escalation and client-centered approaches.
  • Knowledge of behavior management principles and strategies for supporting individuals with brain injuries.
  • General understanding of computer hardware and software, with proficiency in relevant systems and tools.
  • Strong ability to assess and determine effective compensatory strategies for individuals with brain injuries, promoting independence and autonomy.
  • Valid License and access to reliable transportation, with a commitment to maintaining a safe and healthy work environment.
