Job Summary

We are seeking a skilled Nurse Coordinator to join our team at New Brunswick Extra-Mural (EMP)-Programme extra-mural (PEM) du Nouveau-Brunswick. As a key member of our healthcare team, you will be responsible for coordinating patient care and collaborating with interdisciplinary teams to deliver high-quality services.

Key Responsibilities
  • Coordinate patient care by assessing, planning, implementing, and evaluating patient needs in collaboration with healthcare professionals.
  • Collaborate with interdisciplinary teams to ensure seamless delivery of patient care and services.
  • Develop and maintain relationships with patients, families, and healthcare professionals to ensure effective communication and care coordination.
  • Provide education and support to patients and families on healthcare-related topics and services.
  • Participate in quality improvement initiatives to enhance patient care and services.
Requirements
  • Diploma in Nursing or Bachelor Degree in Nursing from an accredited institution.
  • Registration with the Nurses Association of New Brunswick or ability to register.
  • Minimum of two years of specialized nursing experience in family practice, surgical, palliative care, or oncology.
  • Knowledge of the target population of Extra-Mural Services and available community services.
  • Valid New Brunswick Class 5 Driver's License and good driving record.
  • Clean criminal record check and current CPR Level C certification.
  • Excellent communication and interpersonal skills with ability to work efficiently as a team member.
  • Ability to maintain confidentiality and demonstrate a high regard for professional appearance.
