Job Description


This is an exciting opportunity for a Geriatric Care Specialist to join our team at Vancouver General Hospital. The EDiCare program is a unique team-based Geriatrics program implemented in the Emergency Department.



Key Responsibilities:



  • Develop and implement safe discharge plans in collaboration with acute care services, community primary care physicians and community/Home Health services.
  • Provide patient-centered care and support the Emergency Department in managing complex vulnerable Geriatric patients.
  • Liaise with relevant community primary care providers and specialists to support safe discharge plans.
  • Communicate clearly with patients, families, and other healthcare providers.


Required Skills and Qualifications:


To be successful in this role, you will need:



  • A current College of Physicians of BC (CPSBC) License - Full
  • Canadian Medical Protective Association (CMPA) – Mandatory
  • Canadian College of Family Practitioners (CCFP) – Mandatory
  • Geriatric interest and/or training
  • Excellent communication skills
  • Experience working as an interdisciplinary team member
