Job Summary

As a Home Health Licensed Practical Nurse Liaison with Fraser Health, you will play a vital role in facilitating the discharge planning process from acute care facilities to community care services. Your expertise in nursing care and liaison skills will enable you to assess and monitor client needs, develop and implement care plans, and collaborate with interdisciplinary teams to ensure seamless transitions.

Key Responsibilities

  • Assess and monitor client needs, including physiological, psychological, socio-cultural, and spiritual needs
  • Develop and implement care plans in collaboration with interdisciplinary teams
  • Facilitate discharge planning from acute care facilities to community care services
  • Provide support and education to clients and their families
  • Collaborate with healthcare teams to ensure continuity of care

Qualifications

  • Graduation from an approved program of Practical Nursing
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives
  • Valid BC Driver's license and access to a personal vehicle for business-related purposes
