Job Title: Home Health Licensed Practical Nurse Liaison

Job Summary:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Home Health Licensed Practical Nurse Liaison. In this role, you will work closely with our interdisciplinary team to provide high-quality patient care and facilitate smooth transitions between acute care and community-based services.

Key Responsibilities:

  • Assess and monitor patients' needs, including physiological, psychological, socio-cultural, and spiritual needs.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Provide education and support to patients and their families to promote health and well-being.
  • Collaborate with community-based services to ensure seamless transitions and continuity of care.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.

Requirements:

  • Graduation from an approved program of Practical Nursing.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Valid BC Driver's license and access to a personal vehicle for business-related purposes.
