Job Summary
Halton Healthcare is seeking a skilled Registered Practical Nurse to join our team in providing high-quality care to complex transitional care populations. As a key member of our healthcare team, you will be responsible for delivering patient-centered care in a dynamic and rapidly changing environment.
Key Responsibilities
- Provide direct patient care to complex transitional care populations
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Administer medications and perform physical assessments as required
- Participate in wound care and enteral feeding programs
- Maintain accurate and detailed electronic documentation
- Contribute to a positive and supportive team environment
Requirements
- Current registration with the College of Nurses of Ontario
- Current Heart & Stroke Approved BLS Provider Card
- Experience working with and advocating for the elderly
- Medication administration certification and experience
- Physical assessment and wound care skills
- Enteral feeding experience and maintenance of IV lines
- Experience with electronic documentation
- Satisfactory employment and attendance record
