Key Responsibilities- Provide direct patient care to complex transitional care populations, including assessment, planning, implementation, and evaluation of care.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Conduct physical assessments and provide wound care, enteral feeding, and maintenance of IV lines.
- Utilize electronic documentation systems to record patient information and communicate with healthcare teams.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Current registration with the College of Nurses of Ontario.
- Current Heart & Stroke Approved BLS Provider Card upon hire.
- Experience working with and advocating for the elderly.
- Medication administration certification and experience.
- Physical assessment and wound care skills.
- Enteral feeding experience and maintenance of IV lines.
- Experience with electronic documentation systems.
- Satisfactory employment and attendance record.
