Overview

CarePartners is seeking a skilled Community Nurse to join our team. As a Community Nurse, you will have the opportunity to work in a variety of settings, including in-home, clinic, retirement home, or school, providing exceptional nursing services to patients. You will be responsible for developing and maintaining effective Care Plans with respect to Community Health Services, working with patients to provide exceptional nursing services, and communicating and liaising with patients, family members, and others within the interdisciplinary care team.

Requirements

  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • A strong desire and commitment to making a difference in the lives of our patients
  • Exceptional interpersonal skills
  • A valid driver's license and vehicle as regular travel is required
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record including 2 step TB test
  • To protect our patients, we require a current vulnerable sector check and a clear background check
