We are seeking a skilled Registered Practical Nurse to join our team at Lakeland Long Term Care Services Corporation. As a key member of our healthcare team, you will provide high-quality nursing care to our residents in a supportive and dynamic environment.
Responsibilities- Provide nursing services to patients based on individualized care plans, utilizing your expertise and judgment to ensure optimal patient outcomes.
- Collaborate with the healthcare team to monitor patient progress, evaluate the effectiveness of interventions, and make adjustments as needed.
- Conduct thorough assessments of patients' physical and emotional needs, developing and implementing plans to address these needs.
- Work effectively in a fast-paced environment, prioritizing tasks and managing time to meet deadlines and ensure seamless care delivery.
- College diploma or certificate in Practical Nursing, with a strong foundation in gerontology and chronic care.
- Experience in long-term care or a related field, with a proven track record of delivering high-quality patient care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
