Job Title: Rapid Response Nurse

About the Role:

We are seeking a skilled and compassionate Rapid Response Nurse to join our team at Home and Community Care Support Services. As a key member of our Rapid Response Nursing team, you will work with medically complex children and adults, providing hands-on care and support to ensure a smooth transition from acute care to home care.

Key Responsibilities:

  • Screen potential patients for program eligibility in hospital
  • Confirm scheduling of outstanding medical tests and ensure new prescriptions are filled
  • Review medication protocol with patients and caregivers, and answer any questions
  • Provide direct care to patients in collaboration with LHIN Care Coordinators or Service Providers
  • Identify patients requiring accelerated assessment and home care services, and facilitate the home assessment visit
  • Support LHIN Care Coordinators in developing patient care plans and ensuring a smooth transition to ongoing care teams

Requirements:

  • Membership in good standing with the College of Nurses of Ontario
  • Registered Nurse (BScN or diploma) in good standing with the College of Nursing
  • Minimum of five years of experience as a Registered Nurse (BScN or diploma)
  • Working knowledge of community resources and roles of healthcare professionals
  • Emergency/critical care and community nursing experience an asset
