The ideal candidate will have a graduate degree in a relevant field, such as nursing or social sciences, and a minimum of two years of experience in a related field. You will also have excellent communication and organizational skills, as well as the ability to work independently and as part of a team.

Key Responsibilities

  • Assess the need for and coordinate the delivery of Home Care Services
  • Work with patients, families, and healthcare providers to develop and implement care plans
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed
  • Collaborate with other healthcare professionals to ensure seamless care transitions
  • Document patient information and care plans in a timely and accurate manner

Requirements
