Geriatric Nurse Clinician Job Description

We are seeking a highly skilled Geriatric Nurse Clinician to join our team at Surrey Memorial Hospital in Surrey, BC. As a key member of our interprofessional team, you will provide consultation and clinical expertise related to acutely ill older adults who present to Emergency, are admitted to medical/surgical areas, or are referred to specialized clinics.

Key Responsibilities:
  • Provide gerontological nursing assessments and ongoing evaluation of older adults with complex care needs
  • Role model direct care skills to staff and students, and provide just-in-time education at the point of care
  • Collaborate with the interprofessional team to develop and support individualized care plans, and coordinate referrals and linkages to appropriate community resources
  • Evaluate the effectiveness of education programs and consultations to ensure older adult care/learning needs are met
  • Participate in quality improvement and risk management activities, and contribute to the review, development, implementation, and evaluation of evidence-informed clinical decision support tools/policies
Requirements:
  • Three (3) years of recent related clinical experience in acute gerontological nursing, including one (1) year of education and/or leadership experience
  • Completion of post-graduate coursework and/or certification applicable to gerontological nursing (Canadian Nurses Association Gerontology Nursing Certificate)
  • Clinical proficiency/expertise in acute gerontological nursing
  • Ability to role model direct care skills, coach, mentor, and teach nursing skills that are compliant with current standards of practice
  • Ability to communicate effectively both verbally and in writing
