Job Summary

Vancouver Coastal Health is looking for a dedicated Community Care Assistant to join our Home Support Team in Vancouver, BC.

As a Community Care Assistant, you will work under the direction of the Supervisor/Coordinator/Manager or delegate to provide home support services to clients with varied levels of care. You will observe and report significant changes in client presentation, collaborate with the healthcare team, clients, and their caregivers to maximize independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).

Key Responsibilities:

  • Assist clients with daily activities, such as bathing, dressing, and grooming.
  • Provide emotional support and motivation to clients to encourage independence.
  • Collaborate with the healthcare team to develop and implement care plans.
  • Provide input and feedback in the care planning process.

Qualifications:

  • Graduation from a recognized Health Care Assistant Program.
  • Current/Active Registration with BC Community Health Worker and Care Aide Registry.
  • Experience interacting with persons with dementia, geriatric, and/or palliative population will be an asset.
  • Valid BC Driver's license and access to a vehicle, as local area travel may be required.
