We are seeking a highly skilled Pharmacist I to join our team at Alberta Health Services. As a key member of our Pharmacy Services team, you will provide high-quality direct patient care and support drug distribution functions.
Key Responsibilities- Provide direct patient care for acute care beds and LTC beds
- Support drug distribution functions with the technician and/or assistant team
- Participate in interdisciplinary patient care teams and rounds
- Collaborate with patients, physicians, and the interdisciplinary team to optimize patient care
- Utilize Additional Prescribing Authorization to optimize patient medication regimens
- Optimize therapy for acute and chronic disease management
- Empower patients in managing their medical conditions through promotion of healthy lifestyle choices, education, and direct patient care
- Facilitate smooth transitions between care settings
- Participate in professional practice development and growth with support from dedicated Clinical Practice Leaders and pharmacy managers
- Bachelor of Science in Pharmacy
- Active or eligible for registration with the Alberta College of Pharmacy (ACP)
- Additional Prescribing Authority (APA) or awaiting results from APA application
- Prac ID for lab ordering
- Recent direct patient care experience (at least 1 year within the last 3 years)
- Recent Rural Hospital experience working with multidisciplinary teams
- Ability to work effectively in a team environment and independently
- Ability to use evidence-based medicine in direct patient care
- Computer literacy skills, good time management, capable of making decisions, and working independently & cooperatively in team environments
