The estimated salary range for this position is $80,000-$110,000 per annum, commensurate with experience.

Job Description

As a Registered Nurse (RN) or Registered Practical Nurse (RPN), you will play a vital role in providing and coordinating home health nursing services to diverse individuals, families, and groups. Your key responsibilities will include:

• Assessing, monitoring, and making care planning/coordinating decisions
• Evaluating care plan interventions through ongoing reassessment and evaluation results
• Ensuring the quality of care by identifying and acting on factors that affect practice autonomy and delivery of quality care
• Demonstrating professionalism, leadership, judgment, and accountability in interactions with clients and families

Required Skills and Qualifications

To excel in this role, you must possess the following skills and qualifications:

• Registration and good standing with the College of Nurses of Ontario (CNO)
• Membership of your Professional Association (RNAO/WeRPN) preferred
• RN diploma/degree or RPN diploma from a recognized university or college
• Valid First Aid or CPR Certification
• Moderate level of wound care or venipuncture knowledge and experience an asset
• Acute Care and Medical-Surgical nursing experience an asset
• Knowledge of community nursing an asset
• Effective communication (written and verbal) and documentation skills
• Reliable attendance, punctuality, and availability for the role
• Commitment to modeling lifelong learning and professional growth
• Critical thinking skills to provide client care
• Valid Ontario driver's license (G License) and reliable vehicle for traveling
