Community Health Specialist

We are seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Community Nurse. As a Community Nurse, you will have the opportunity to work in a variety of settings, including in-home, clinics, retirement homes, and schools, providing exceptional nursing services to patients of all ages.
Key Responsibilities- Develop and maintain effective care plans with respect to community health services, independently or in cooperation with the Ontario Health atHome.
- Provide nursing services in a variety of settings, including in-home, clinics, retirement homes, and schools.
- Communicate and liaise with patients, family members, and other members of the interdisciplinary care team.
- Perform other clinical duties as required.

- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma.
- Current College of Nurses of Ontario (CNO) registration in good standing.

- Exceptional interpersonal skills.
- A valid driver's license and vehicle, as regular travel is required.
- Excellent English verbal and written communication skills.
- CPR Certification.
- Current immunization record, including a 2-step TB test.
- To protect our patients, we require a current vulnerable sector check and a clear background check.
