Job SummaryWe are seeking a Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Community Health Nurse. In this role, you will work independently or in cooperation with the Ontario Health atHome to develop and maintain effective care plans for patients receiving community health services.
Key Responsibilities- Develop and maintain effective care plans for patients receiving community health services
- Provide exceptional nursing services to patients in their homes
- Perform a wide range of clinical duties, including compression therapy, oncology support, hydration management, IV therapy, ostomy and wound care, catheter management, and more
- Work collaboratively with other healthcare professionals to ensure seamless care delivery
- Current College of Nurses of Ontario (CNO) registration in good standing
- Strong desire and commitment to making a difference in the lives of our patients
- Exceptional interpersonal skills
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record, including 2-step TB test
- Vulnerable sector check and clear background check
CarePartners is proud to serve the community through clinics, transitional care units, and relief in retirement homes and shared care settings. Our Community Nursing Services outreach program has been organizing staff-led medical care and clinics in countries with poor access to healthcare since 2009.
