Job Summary

We are seeking a highly skilled and compassionate Field Case Manager to join our team at CB Canada. As a Field Case Manager, you will play a critical role in providing exceptional patient support and navigation services to our clients.

Key Responsibilities
  • Patient Support
    • Welcome and onboard patients to our program, capturing essential patient information
    • Provide therapy guidelines to patients and healthcare professionals
    • Coordinate private and public coverage activities with insurers, ensuring timely access to therapy
    • Follow up with patients on reimbursement navigation and financial assistance
    • Assist patients with pharmacy selection and coordinate drug delivery
    • Monitor patient therapy and report adverse events
  • Mentorship and Peer Support
    • Assist in training and development of new Field Case Managers
    • Support and assist peers through regular communication and collaboration
  • Client Interface
    • Participate in scheduled meetings with healthcare professionals and office staff
    • Develop strong working relationships with healthcare professionals and office staff
    • Provide assistance to resolve client issues and concerns
    • Attend Continuous Medical Education events and report new activities
  • Other
    • Complete all relevant reports and documentation as per specified timelines
    • Perform other duties and tasks as required by the company
Requirements
  • Education and Experience
    • RN or RPN registration with the Provincial College of Nurses
    • Minimum three (3) years of nursing experience in a clinical setting
    • Available to travel within an assigned territory, with some overnight travel required
  • Skills and Knowledge
    • Strong customer service and communication skills
    • Ability to analyze and interpret regulations and legislation
    • Fluency in English and/or French may be required depending on program and geographic location
