Job Summary

We are seeking a highly skilled and experienced Field Case Manager to join our team at Innomar Strategies. As a Field Case Manager, you will be responsible for managing the patient journey, providing exceptional customer service, and ensuring that patients receive the best possible care.

Key Responsibilities

  • Manage patient relationships and provide ongoing support to ensure patient satisfaction and retention
  • Collaborate with healthcare providers, payers, and other stakeholders to ensure seamless patient care
  • Develop and implement patient care plans to meet individual patient needs
  • Provide education and support to patients and their families on disease management and treatment options
  • Monitor patient outcomes and make adjustments to care plans as needed
  • Stay up-to-date on industry trends, best practices, and regulatory requirements

Requirements

  • Registered nurse (RN) or related degree
  • Experience in healthcare, case management, or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong analytical and problem-solving skills
  • Ability to work in a fast-paced environment and prioritize multiple tasks
