About the Role:
We are seeking a skilled Registered Practical Nurse to join our team as a Community Care Specialist. In this role, you will provide high-quality nursing care to patients in their homes, working collaboratively with our team to ensure the best possible outcomes.
Key Responsibilities:
- Provide scheduled visits to patients in their homes, assessing their needs and developing care plans to meet their individual requirements.
- Deliver safe and effective nursing care, using appropriate assessment tools and techniques to identify patient needs.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
- Document patient interactions and care plans, maintaining accurate and up-to-date records.
- Participate in ongoing education and training to enhance clinical skills and knowledge.
Requirements:
- Registration with the College of Nurses of Ontario as a Registered Practical Nurse in good standing.
- Valid driver's license and access to a reliable vehicle.
- Experience in community care or a related field is an asset.
- Excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
- Flexibility and adaptability, with a willingness to travel to patient residences within a designated area.
