Job SummaryThe successful candidate will be responsible for conducting assessments, formulating goals and recommendations, planning and implementing client-centered treatment programs, and developing themselves through ongoing education and training.
Key Responsibilities- Conduct comprehensive assessments to identify client needs and develop effective treatment plans
- Formulate goals and recommendations that align with client objectives and priorities
- Plan and implement client-centered treatment programs that promote independence, autonomy, and overall well-being
- Develop and maintain strong relationships with clients, families, and healthcare professionals to ensure seamless care coordination
- A competitive salary of $83,500 - $110,000 per year, depending on experience
- A comprehensive benefits package, including health, dental, and vision coverage
