Job Summary

We are seeking a dedicated and compassionate Mental Health Support Professional to join our team at JAZ Community Support Limited. As a key member of our community support team, you will provide essential mental health services to individuals in need, working closely with our clients to address their unique challenges and promote overall well-being.

Responsibilities
  • Assess client needs and develop personalized support plans to meet their physical, emotional, and social requirements.
  • Collaborate with other healthcare professionals and community agencies to ensure comprehensive care and coordinate referrals as needed.
  • Provide education and training on healthy living skills, stress management, and coping strategies to empower clients to make informed decisions about their lives.
  • Maintain accurate records of client interactions, progress, and outcomes, utilizing data-driven insights to inform service delivery and quality improvement initiatives.
  • Foster strong relationships with clients, families, and caregivers, promoting trust, respect, and open communication.
Requirements
  • A minimum of 1-year experience in a related field, preferably in community mental health or social services.
  • A valid Mandt System certification, Non-Violent Crisis Intervention (NVCI) Certificate, and First Aid/CPR certification.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and teams.
  • Demonstrated proficiency in electronic mail, MS Office, and other relevant software applications.
  • Ability to lift up to 13.5 kg (30 lbs) and work in various settings, including community agencies and clients' homes.

  • A competitive salary range of $45,000 - $55,000 per annum, depending on experience.
