Job Description

We are seeking a skilled and compassionate Nurse Coordinator to join our team at the Cree Board of Health and Social Services of James Bay (CBHSSJB). As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to patients with chronic kidney disease.

About Us

The CBHSSJB is responsible for delivering health and social services to the Eeyou Istchee region, home to nearly 18,000 people from the Cree Nation. Our organization is committed to providing front-line and second-line services through our local Community Health Centers and regional hospital centre.

Key Responsibilities
  • Evaluate and monitor patients' health conditions, developing and implementing individualized care plans to ensure optimal outcomes;
  • Collaborate with healthcare professionals, including nephrologists, to provide comprehensive patient care and support;
  • Provide education and guidance to patients and their families on chronic kidney disease management and treatment options;
  • Facilitate communication between patients, families, and healthcare teams to ensure seamless care coordination;
  • Participate in research activities aimed at improving health outcomes and preventing disease;
  • Work under the supervision of the DPSQA-Health, collaborating with various stakeholders, including external collaborators like the MUHC nephrology team and internal collaborators such as specialized services and case managers.
Requirements
  • Member in good standing of the O.I.I.Q;
  • Two years of experience as a nurse;
  • Minimum one year of experience working with chronic kidney disease patients;
  • Excellent communication, organizational, and critical thinking skills;
  • Knowledge of software used for clinical patient monitoring, including Nephrocare and Medipatient;
  • Fluency in English; fluency in Cree or French is an asset.
Benefits

We offer a competitive salary range of $$83,419 - $94,119 per year, depending on experience, plus benefits that include:

  • Remoteness premium: $14,000 per year;
  • Cargo premium: A weekly amount to be confirmed based on community and family status;
  • Accommodation provided: Furnished, heat, and electricity included;
  • Moving and storage expenses paid;
  • Three to four annual sorties depending on family status;
  • Group insurance plan;
  • Defined-benefit pension plan (RREGOP).
