Key Responsibilities- Coordinate and prepare telehealth and on-site appointments for specialists, ensuring seamless communication and collaboration with healthcare teams.
- Review medical records, identify areas for improvement, and implement effective follow-up strategies to ensure optimal patient outcomes.
- Act as a liaison between patients, families, and healthcare professionals, facilitating open communication and coordination of care.
- Monitor and evaluate patient progress, updating interventions and care plans as needed in collaboration with the Community Mental Health Centre (CMC).
- Participate in research activities aimed at promoting health and preventing disease, contributing to the development of evidence-based practices.
- Member of the Order of Quebec Nurses (O.I.I.Q.)
- DEC in Nursing
- Two (2) years of experience as a nurse
- Fluent in English; Cree or French is an asset
